Publicity Release Agreement

I hereby grant Oregon Workforce Partnership AKA Oregon Employment and Training Association, hence known collectively as “OWP” permission to use my name, likeness, voice, and biographical material in a photograph, video, audio, written format, and/or any other medium about me (from here on known as personal images and information) in any and all of its publications, including but not limited to website, recordings and all other social media forums. I consent to this without payment or any other consideration. I understand and agree that these materials will become the property of OWP and will not be returned.

I hereby irrevocably authorize OWP, Inc. to edit, alter, copy, exhibit, publish or distribute all personal images and information for purposes of publicizing OWP or for any other lawful purpose. I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph or information.

I hereby hold harmless and release and forever discharge OWP from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
